*Adoption Agreement*

Please read carefully and understand all statements.

This is a legal binding document.

                                  (For future reference Lilac Groves Pampered Pups will be known as LGPP.)

1.) Non-Breeding: It is understood the above animal will never be used for breeding. The above animal will be Spayed/Neutered at an age appropriate time, determined by a licensed veterinarian & Lilac Groves Pampered Pups.(INITIALS) __________

2.) Health: The above animal will be placed on an inoculation program as prescribed by a licensed veterinarian in order to assure & maintain the health of the above animal. LGPP is NO Longer Medically Responsible for the care of the said animal.(INITIALS)_________

3.) Care: The owner as stated above accepts full responsibility for maintaining a reasonable level of care for the animal above including food, water, grooming, exercise, training, attention, safety, humane treatment & shelter. The adopter as stated above will ensure the animal stated above will receive proper medical care & is financially able to do so.(INITIALS)________

4.) Transfer of ownership: The animal stated above will not be transferred ;( sold, fostered, traded, given away) to any other person/persons or humane society/shelter unless prior approval has been granted from LGPP in writing.(INITIALS)_________

5.) Reservation of Rights: LGPP reserves the right to an ongoing review of this adoption including but not limited to:  follow up on any complaints or reports to protect the welfare of this animal. If the terms of this agreement are not satisfactory or upheld by the owner, & /or if any misrepresentations have been made to LGPP in order to obtain the said animal, LGPP reserves the right to void the contract and demand the return of the animal stated above. If for any reason legal fees are incurred by LGPP in order to obtain custody of said animal, said owner will be responsible for any and all fees.(INITIALS)_________

6.) Insurance: I understand I am required to apply for(within 24hrs of adoption) & obtain shelter insurance for the animal stated for at least one month.(INITIALS)_________ www.sheltercare.com

7.) Housing: If adopter resides in housing which is rented, adopter shall obtain permission from the landlord prior to the animal's placement with the adopter. A copy of the lease approving pets in the rental housing will be acceptable proof.(INITIALS)_________

8.) Adjustment time: Adopter understands that animals typically require time to adjust to their new homes & surroundings. This includes attitude & housebreaking adjustments.(INITIALS)_________

9.) Adopter shall not hold "LGPP" liable or responsible for any future medical care or treatment for the adopted animal.(INITIALS)_________

10.) It is expressly understood by the parties that "LGPP" makes NO Representations, OR warranties about the animal whatsoever, including the following but not limited to: Health, Age, Temperament, Longevity, Color, Size, Breed or compatibility with any other person(s), animal(s) or thing(s), or the animals ability to reasonably adjust to adopters home.(INITIALS)_________

If any legal action or any arbitration or other proceeding is brought for the enforcement of this  agreement, or because of an alleged dispute, breach, default or misrepresentation in connection with any of the provisions of this agreement, LGPP shall be entitled to recover reasonable attorney's fees & other costs incurred in that action or proceeding, in addition to any other relief which LGPP may be entitled to. This agreement shall be construed in accordance with, and governed by, the laws of the state of New Hampshire. In addition to the forgoing, owner agrees to not hold LGPP responsible from any claim or suit filed by any third party & said hold harmless shall include defense costs including attorney's fees if incurred in the defending of any such claim or action.(INITIALS)_________

It is understood that this is an adoption, of the said animal & the Adoption fee is NON-REFUNDABLE.(INITIALS)_________

This is a legally binding contract. No provisions of this contract may be altered, modified or in any way changed unless the same is in writing. This contract contains the entire agreement by & between the parties.(initials)______

I understand that this is a rescue. There is No history, No background on the said animal. I understand I am to take this animal to a vet within 48 hours of date of adoption, and if at the time of the exam a life threatening disease is found the animal is to be RETURNED to LGPP with a letter from the examining veterinarian within 24 hours of the veterinarian visit  for a refund of the purchase price only.(initials)________

NH state law gives a 2 week guarantee on the health of the animal, during that 2 week period; NO medical treatments, testing, medicating etc. are to be done unless agreed upon by LGPP. ALL procedures must be cleared with LGPP first. Without consent, contract is NULL and VOID. And any and all expenses are yours.(initials)________

I hereby state that I have read this contract in its entirety & agree with and fully understand the conditions therein. IF THIS CONTRACT IS NOT FOLLOWED LGPP HAS THE RIGHT TO REPOSSESS THE SAID ANIMAL AT ANY TIME.
